Hospital admission and discharge planning

If someone you care for is admitted to hospital, there will be a need to consider how they will be supported when they are ready to return home. For example, their needs may have changed or they may require additional support.

You will need to consider the type and amount of support you can provide along with help that may be required. For instance, you may be able to help with shopping and meals, but perhaps both of you would prefer someone else to support with other tasks e.g. personal care.

Being identified as a carer

You can ensure that you are recognised as a carer – you can get one of our carer’s cards which also can also act as the card for your What If? Plan which can be activated if you have an emergency meaning you cannot carry out your caring role. Both Cambridge University Hospitals NHS Foundation Trust (covering Addenbrooke’s Hospital and The Rosie) and North West Anglia NHS Foundation Trust (covering Peterborough City Hospital and Hinchingbrooke Hospital) have carers passports in place – see the information below.

Being identified as a carer will help you and the person you care for if they are admitted to hospital. Having a What If? Plan will also be very helpful if something happens that means you are taken into hospital.
